Summary:
Responsible for connecting expecting parents and parents with children 0-5 to community resources and early childhood programs.
Completes assessments to identify the needs of families receiving medical care through LCDF and makes referrals as needed.
Participates in Health Fairs organized by LCDF and the community to recruit expecting parents to receive medical care through LCDF.
Conducts ongoing visits to Mountain View and works closely with Women's and Pediatrics providers and personnel.
Core Competencies:
Bilingual English and Spanish preferred Strong interpersonal and communication skills Self-initiative and adaptability High degree of organization and attention to detail Ability to maintain a high degree of confidentiality Strong knowledge of community resources Demonstrate ability to work with Windows programs and to utilize technology to perform job responsibilities.
General office skills, including outstanding phone etiquette.
Maintains a professional image.
Must be able to work flexible hours Provides excellent customer service to internal customers and greets visitors in a friendly, helpful, and courteous manner.
Job Requirements:
High School Diploma or Equivalent required.
Community Health Worker Certification is preferred.
Opportunity to obtain CHW certification and Infant Mental Health Endorsement within one year of employment.
